Analysis of Breast Cancers at Previous Screening Mammography

Abstract
Purpose: We analyzed the previous screening mammography in patients with breast cancer to reduce false negative screening mammography.

Materials and Methods: Between October 2003 to July 2009, 47 patients with breast cancer who underwent previous screening mammography in our institution were included. We retrospectively reviewed the previous screening mammograms and classified them into false negative, minimal sign, true negative, true positive and occult cancer. We also analyzed the attributing factors of false negative screening mammography including misperception and misinterpretation.

Results: Of the 47 cases, false negative case was found in 19 (40%), minimal sign in 4 (9%), true negative in 10 (21%), true positive in 2 (4%) and occult cancer in 16 (34%). False negative mammographic findings were microcalcifications in 8 (42%), asymmetry in 7 (37%), architectural distortion in 3 (16%), and nodule in 3 (16%). Misperception was more common than misinterpretation of the false negative mammography. The attributing factors of misperception were dense breast, lesions seen on only one view, lesions located at edge of glandular tissue or image, small size, amorphous microcalcifications, low density of the lesion, no serial review, and lesions surrounding scattered fibroglandular tissue. The attributing factors of misinterpretation were no strict application of BIRADS, small size, slow growing cancers, microcalcifications associated with diffuse benign microcalcifications and no serial review.

Conclusion: To reduce false negative screening mammography, we need careful analysis, serial review of previous examination, and strict analysis of BIRADS.
